- You must have a physical or mental disability
- Your disability must keep you from getting and/or keeping a job
- Must be 16 years of age or older
- You must be able to care for daily personal needs or provide an attendant
- Your physical or mental condition must be stabilized
- You must be able to ride public transportation or provide own
- Be referred by The Alabama Department of Rehabilitation Services
